Comprehensive Gender Services Program
The Comprehensive Gender Services Program (CGSP) at University of Michigan Health helps transgender, gender nonconforming, and nonbinary adults access and obtain the highest quality gender-affirming medical care.
CGSP acts as a central hub for gender-affirming care at U-M Health. Patients in our program can contact us directly to ask questions, check in on where they’re at in process for receiving care, or to get information on specific services. CGSP also accepts and reviews letters of support that patients may need for clinical services.
Our Approach
Our team believes that identifying with a gender that is different from one’s sex is a normal part of the human experience, and that people with all gender identities—including agender, non-binary and gender queer identities—deserve equal access to the highest quality care. CGSP providers at U-M Health are members of the World Professional Association for Transgender Health (WPATH), and medical and mental health services are provided consistent with the WPATH Standards of Care.
Our team of medical providers includes physicians from the areas below, as well as a team of mental health clinicians includes clinical social workers who have many years of experience working with people of diverse gender identities.
Get Started with CGSP
Patients do not need to have a doctor's referral to get started with CGSP. You may fill out the Service Inquiry Form and follow the instructions on the form. The information will be used to either create a chart for you or update an existing one. CGSP will also send you information packets based on the services you’re interested in. Initiating hormone therapy can affect your fertility in the future. Gender-affirming surgeries involving the reproductive system will result in infertility. If you would like to leave open the option of having biological children in the future, you may wish to consider fertility preservation.

Dermatology at U-M Health is proud to provide laser hair removal services to transgender people. To reach Dermatology, please call 734-615-0682.
Before scheduling a hair removal consultation that’s covered by insurance or required for a vaginoplasty or phalloplasty with dermatology, you may need a letter of support on file with CGSP.

U-M Health provides hormone therapy and puberty blockers as part of our gender affirming care services for patients age 19 and older.
We refer to the following clinic locations and providers within Michigan Medicine for hormone care. Please call these clinic locations directly to schedule your first appointment. You do not need a referral from a mental health care provider in order to start hormones. Call the numbers below to schedule an appointment directly.

Please be advised that some gynecology locations such as the Women's Clinic located in Von Voigtlander Women's Hospital, and the Von Voigtlander Women's Hospital generally, have a particular focus on “women's health” as it relates to reproduction and childbearing. We understand that not all patients wish to be in that environment, but we assure you that our doctors welcome all patients to receive care through the Women's Clinic, regardless of their gender.
U-M Health is proud to provide hysterectomies for transgender people. To access that surgery, please contact any of the offices below and schedule an appointment for a consultation for a hysterectomy.
For hysterectomy and/or vaginectomy (vagina removal) we do not require letters of mental health support by policy. However, most insurance companies require one or two letters of mental health support before they will authorize insurance coverage for hysterectomies and vaginectomies for gender-affirming care. You can schedule your consultation before your letter of support is sent in.

CGSP clinical social workers provide counseling services for transgender, gender diverse, and nonbinary people based on their availability. In the case that we are not taking on new patients you can requested to be added to our waitlist. We can also provide resources for gender affirming therapists in the community.
Services are also provided to people questioning their gender. CGSP clinical social workers are able to conduct evaluations and write letters of support, as appropriate, depending on each individual’s unique goals. To make an appointment, please call 734-998-2150.
Please note that CGSP at Michigan Medicine accepts support letters for medical intervention from fully licensed providers (such as a mental health therapist, psychiatrist, endocrinologist etc.) who are experienced in working with transgender patients and can assess mental health stability.

Outlined below is the process of updating your electronic health record to reflect your preferred name, gender identity, pronouns, and sexual orientation. To do this you will need to have a MyUofMHealth patient portal account set up. Also outlined is the process for updating your legal name. You cannot update your legal name through the patient portal but may contact the Registration Department to do so.
Preferred Name, Gender Identity, Pronouns, & Sexual Orientation
To update your preferred name and gender identity follow these steps:
- Log into MyUofMHealth on either your computer or mobile device.
- Select the Your Menu option.
- Within Your Menu, scroll to the My Record section and select Questionnaires.
- A new window will open called Optional Questionnaires. In that window you will have five questionnaires to pick from. They are labeled Gender Identity, Preferred Name, Sexual Orientation, Poke and Procedure Plan, and Disability Accommodations.
- Fill out as many or as few questionnaires as you would like.
Legal Name
To update your legal name, you will first need to update your legal name with your insurance provider. Once your insurance has been updated, please call the Registration Department at 734-936-4990 or 866-452-9896.

Gender-Affirming Surgery
Letters of Support
Please note that most insurance companies require you to have one or two letters of support from mental health providers prior to obtaining gender-affirming surgeries. Different insurance policies have different letter requirements for different surgeries. If you are unsure how many letters you need, please contact your insurance provider.
Wait Times
Wait times can vary depending on the service you are interested in and our providers' capacity. If you would like an estimate, please call CGSP. You can generally expect the whole process from submitting your support letter to surgery to take 8 months to over a year depending on the procedure. If you are requesting multiple procedures, those processes can be happening at the same time.
Steps
Below is a general list of steps that happen before having a gender-affirming surgery at University of Michigan Health. This might be slightly different for different procedures, but they all generally follow the same process. See the above service listing for more information on your chosen surgery. For more specific information, you can submit a Service Inquiry Form above to connect with our services. If you have already filled out that form you can all us at 734-998-2150 or email us at [email protected] with any questions.
- Submit a Service Inquiry Form (SIQ) to CGSP. After you submit the form, you will hear back from CGSP in the method you requested in 1 to 2 weeks. You will be provided with a list of your next steps and information packets for each of the services you requested. If you requested information be sent through email, make sure that CGSP’s email address is not being filtered into your spam email box. If you requested information to be sent back through the mail add an extra 1 to 2 weeks to the time for it to be processed by CGSP and moved through the mail system.
- Submit your letter of support from a mental health provider. The information packet you received from CGSP will include a checklist of what must be included in that letter. Depending on your insurance provider and surgery you may need two letters of support. Letters of support must be dated within a year of your surgery date. Please make sure your letter lists all the services you're interested in. You do not need an individual letter for each service if you are requesting multiple consultations. If you need assistance finding a letter writer, you can contact CGSP for resources. If you want to call CGSP to confirm we received your letter, please wait a week to do so. That will give your letter time to move through our system to a point where we can confirm we received it.
- CGSP reviews your letter of support to make sure it includes all of the required information being requested by surgeons and insurance providers. Once CGSP has your letter, it will go through an internal review process to prepare it for insurance prior authorization. CGSP wants to make sure your letter includes all the required information. If your letter needs revisions, CGSP will contact your letter writer directly for the revisions. This process can take up to a month depending on CGSP’s capacity and the volume of letters we receive.
- Once your letter of support is reviewed and marked as ready, CGSP will contact you about scheduling a consultation. CGSP’s first preference for contact is through your patient portal. CGSP will send you a registration email for the patient portal during your intake. If you need a new registration email sent to you, please contact CGSP. Be sure to have notifications turned on for your portal account so you do not miss a message from CGSP. If you do not have your patient portal set up, CGSP will reach out by phone using the number listed on your Service Inquiry Form.
After hearing from CGSP about your letter of support, call CGSP to schedule your consultation. Your wait time for consultation will depend on the specific doctor’s schedule.
After your consultation, the rest of the process will be handled by departments outside of CGSP. Your main point of contact will be the department performing the surgery. If you need the contact information for that department, you can contact CGSP for the number.
- After your consultation appointment, your support letter will be submitted to your insurance provider for prior authorization. The length of time it takes for prior authorization to be approved varies by insurance provider.
- Once prior authorization is approved, a surgery scheduler will contact you to schedule your surgery. You can generally expect to have your surgery 3 to 6 months after your consultation.

First Tuesday of every month

The group provides an accepting and inclusive space for parents and parental figures of transgender and gender questioning people to find support and make connections with others. Participant interests direct the discussion.

Partners of Transgender and Gender Diverse People Support Group
This group provides an accepting and inclusive space for partners/spouses of transgender, gender diverse, and gender questioning people, to find support and make connections with others. Participant interests direct the discussion.

You do not need a letter of support from a mental health provider to start hormones. However, most insurance companies require you to have one or two letters of support from mental health providers prior to obtaining gender-affirming surgeries. Different insurance policies have different letter requirements for different surgeries.
Regardless of insurance policy, U-M Health requires all patients having top surgery at least one letter of mental health support, and all patients having orchiectomy, metoidioplasty, vaginoplasty, and phalloplasty to have two letters of mental health support. We do not require letters of mental health support for hysterectomy or vaginectomy by policy; however, most of our patients need two letters of mental health support for these surgeries in order to obtain insurance coverage.
CGSP is not able to recommend particular insurers or plans. If you have questions about your insurance coverage for gender affirming medical care, please contact your insurance company directly. While we can inform people about the insurance plans that are accepted for payment at U-M Health (and most plans are accepted) just because this plan is accepted, doesn't mean that this plan covers all gender affirming medical services. In order to know what your insurance plan will cover, it is necessary to call the insurer and ask them directly. In order to ensure accurate information from the insurer, it is sometimes advisable to obtain a copy of the Plan Documents and/or Summary of Benefits and Coverage.
We are aware that many surgeons post pre- and post-operative photos of patients undergoing gender-affirming surgery. We are also aware that some patients would like to see photos of patients we’ve operated on here at U-M Health. Regardless, we don’t post or share photos of other patients publicly for primarily ethical reasons.
Our ethical concern stems from the fact that surgeons who post pre- and post-operative photos of any procedure (cosmetic procedures, breast reconstruction, gender affirming procedures, etc.) invariably post only their excellent results. Photos of average results are never posted, and certainly sub-optimal results are never posted either. In reality, there is a range of outcomes from poor to excellent for any procedure, so that posting only excellent results presents a skewed representation of expected outcomes and, in our opinion, is misleading and inappropriate.
In addition, any posted photo passes permanently into the public domain and can be copied and propagated, raising concerns about patient confidentiality and potential misuse. There is an abundance of information, including images, of outcomes of gender affirming surgery posted by individual patients. We believe these sources present a more realistic range of outcomes than is found on marketing-focused surgeon websites.
Some surgeons may provide images of past procedures at the time of consultation.
Provider Resources
If you have a patient interested in gender affirming medical care, encourage them to complete the Service Inquiry Form and return it to CGSP. No formal referral is needed for patients to receive or return the form, or to contact CGSP directly. Once CGSP receives the form, our staff will contact the patient to discuss next steps for their requested services.
If you are listed as the patient’s PCP and the patient signs a release of information, Michigan Medicine providers involved in the patient’s care may correspond with you regarding their treatment.

CGSP provides resources to assist letter writers, including a checklist of required assessments and components for letters of support, as well as a preformatted template. Patients will receive these resources in their information packet following submission of the Service Inquiry Form.
