Affiliation Agreement

Partnering with nursing schools nationwide, University of Michigan Health | Michigan Medicine requires a formal Affiliation Agreement and Program Addendum to ensure compliance and a seamless start to clinical placements.

All non-University of Michigan nursing schools must have a valid Affiliation Agreement (Master Contract) and a corresponding All-In-One Program Addendum on file with University of Michigan Health | Michigan Medicine before starting clinicals.

IMPORTANT: Only our approved agreement templates are accepted. Please do not submit your school’s master contract. Minor edits may be reviewed, but significant changes will not.

In addition, only authorized contracting officials from both institutions may negotiate and execute these agreements. This typically does not include instructors or academic advisors unless they also serve as your school’s contract facilitator.

The Process

Master Agreements last 5 years and legally covers all the Program Addendums.

Requirements

  1. You must have a preceptor in place with a start date.
  2. You must follow the deadline timeline.
  3. If all the above is in place, please send the required information to April Stingo at [email protected]. This information is listed below in the "Information Needed" section below.

Timeline & Deadlines

The process of obtaining a contract may take up to 16 weeks depending on the length of time needed for the School of Nursing and the UMH Office of the General Counsel to reach an agreement of terms. Please submit the contract request using the following Deadlines Dates listed below. 

IMPORTANT NOTE: The UMH Office of the General Counsel needs the All-In-One Program Addendum to be date stamped with the same date or a date after the Affiliation Agreement is finalized. Due to the All-In-One Program Addendum being processed in this way by Clinical Placement and not our legal department as one full document process, this may cause delays in student onboarding or clinical start date.

  1. Spring/Summer 2026: Submit by January 19, 2026
  2. Fall 2026: Submit by June 15, 2026
  3. Winter 2027: Submit by September 14, 2026
