Getting Started with HomeMed
Starting home infusion or specialty pharmacy therapy can feel overwhelming. At HomeMed, our team is here to guide you through every step and make the process as simple and comfortable as possible.
You may be referred to HomeMed after a hospital stay, clinic visit, or by your provider. Once your referral is received, our team begins coordinating your care and preparing everything you need to safely receive therapy at home.
After your referral, our care team works behind the scenes to set up your therapy:
- Review your referral and confirm your treatment plan with your provider
- Verify insurance coverage and obtain any required approvals
- Coordinate nursing services if needed
- Prepare your medications, supplies, and equipment for home use
Our goal is to ensure everything is ready before your therapy begins.
Your therapy is tailored to your specific needs. Before starting treatment, our team considers important factors such as your living situation, support system, and ability to manage therapy at home.
This helps us create a plan that is safe, effective, and manageable for you and your caregivers.
HomeMed prepares and delivers your medications, supplies, and equipment directly to your home. Medications are packaged carefully to maintain safety and quality, including temperature-controlled delivery when needed.
We will coordinate delivery timing with you and provide instructions to ensure you are ready to begin therapy.
Before you start therapy, you and your caregivers will receive education and training from our team or your assigned nursing agency. This may include:
- How to administer your medication
- How to use infusion pumps or equipment
- How to care for your IV line or feeding tube
- What to watch for and when to call for help
Our goal is to make sure you feel confident and prepared.
HomeMed continues to support you throughout your therapy:
- Regular clinical monitoring and follow-up
- Coordination with your provider and care team
- Lab monitoring and therapy adjustments as needed
- Ongoing education and support
Our clinicians are available 24 hours a day, 7 days a week to answer questions and assist with urgent needs.
Your care is supported by a multidisciplinary team, which may include:
- Pharmacists
- Nurses
- Dietitians
- Care coordinators and support staff
Together, we work closely with your provider to ensure your therapy is safe, effective, and personalized to you.
You should contact HomeMed if you have questions or need help during your therapy. Our team is here to support you at any time.
