How to Make a Referral

For University of Michigan Physicians, a referral can be made by contacting your Care Manager or by using the MiChart Referral Order Processing. To make a referral from a hospital other than University of Michigan Hospital, you may call our Intake Department at 734-975-7441 or 800-862-2731 (extension 7441). Information that will be needed in order to process the referral include: 

  • Patient Name 

  • Address of care and phone numbers 

  • Date of birth 

  • Insurance information 

  • Type and start of therapy 

  • Name, phone number of the following physician for home care orders 

  • Visiting nurse agency name and phone number

Enteral referrals from a hospital other than University of Michigan, refer to the instruction sheet prior to completing the Enteral Order Form. To avoid delays in processing, ensure the form is complete with all relevant information and is signed by the following physician.
