HomeMed provides home infusion and enteral nutrition services for adult and pediatric patients. As part of Michigan Medicine, our pharmacists and care team work closely with your providers to support your care at home and during the transition from hospital to home. We also bill most insurance plans, Medicare, and Medicaid directly, and our team helps manage the related paperwork.
HomeMed helps coordinate your transition from the hospital or clinic to home therapy. Our team works with your doctor and other care providers to arrange the appropriate equipment, IV access, supplies, and support for safe care at home.
HomeMed clinicians customize your care plan and monitor your therapy throughout treatment. This may include coordinating lab work, review results, and communicating with your care team as needed.
HomeMed prepares your medications, equipment, and supplies according to your doctor’s orders. Deliveries are scheduled based on your location and care needs, and we may contact you or your caregiver before delivery to confirm supplies, review needs, and ensure medications are packaged safely, including refrigeration when required.
