Letters of Mental Health Support

You do not need a letter of support from a mental health provider to start hormones. However, most insurance companies require you to have one or two letters of support from mental health providers prior to obtaining gender-affirming surgeries. Different insurance policies have different letter requirements for different surgeries. Regardless of insurance policy, Michigan Medicine requires all patients having top surgery at least one letter of mental health support, and all patients having orchiectomy, metoidioplasty, vaginoplasty, and phalloplasty to have two letters of mental health support. We do not require letters of mental health support for hysterectomy or vaginectomy by policy, however, most of our patients need two letters of mental health support for these surgeries in order to obtain insurance coverage. For more details, please find more detailed information listed under surgeries of interest on the left-hand side of this page.
