What is proxy access

The MyUofMHealth Patient Portal has a function called Proxy access. This allows a parent, guardian or other designated individual (like a spouse, family member, or friend) to connect to the information regarding a family member or friend. If you are a parent who wants to use the portal to view your child’s health information, learn more about this type of proxy access.

Frequently Asked Questions

How do I get proxy access to my child?

Proxy access allows parents and legal guardians to access health information for your child by connecting your personal MyUofMHealth portal to the child or diminished capacity adult that you represent. Proxy is used by first logging into your own account, not that of the patient. Once logged into your account, you will be able to see a tab dedicated to the patient whose information you are able to see. Because of the sensitive nature of medical information, each adult must establish his or her own MyUofMHealth Patient Portal account, and partners cannot share access without requesting proxy access.

  1. From Your Menu in your portal and select Request Access to Minor’s Record.
  2. Enter your child’s information and submit your request.
  3. You will receive a notification to confirm your proxy when your request is approved.

If you are not a patient at U-M Health or are legal guardian / DPOA for another adult patient, use this online form to request access to request proxy access.

How do I use proxy access?

Once you have obtained proxy access to another patient’s portal information, you may ask questions regarding their health using secure messaging. The questions you ask become part of the patient’s medical record, so it is important you ask only questions about their health while viewing their information through proxy. This helps make sure your questions are sent to the correct health care provider.

How do I give proxy access to my partner, family member, or friend?

You can give trusted family members and friends access to your medical records when you need help managing your care. Because of the sensitive nature of medical information, each adult must establish his or her own MyUofMHealth Patient Portal account, and partners cannot share access without granting proxy access.

  1. From Your Menu, select the Sharing Hub activity in your portal, then choose Manage Friends and Family Access.
  2. Select Invite friends or family and enter the name and email address for the person you want to invite.
  3. After they receive the email and confirm that they know you, they can access your information through their own MyUofMHealth account.

If you are unable to sign up for your own account, a paper version of the adult proxy request form can be used.

How do I remove a proxy?

Adult patients can remove proxy access at any time by logging into your account and removing the proxy. The removal will occur immediately.

  1. From Your Menu, select the Sharing Hub activity, then choose Manage friends and family access.
  2. Find the name of the person you want to remove, then select Remove.
  3. Confirm you want to remove access when prompted.
