- Go to the MyUofMHealth homepage or your MyUofMHealth mobile app.
- Select "Forgot username?" or "Forgot password" and follow the instructions.
You can find your medical record number (MRN) on your After Visit Summary, your hospital wristband, or on most print-outs from your doctor’s office. Your MRN is different from you student ID and billing account number.
Proxy access allows parents and legal guardians to access health information for their child by connecting their personal MyUofMHealth portal to the child they represent.
Option 1: If you already have a personal MyUofMHealth portal, log in to your portal and select Request Access to Minor’s Record from Your Menu.
Option 2: If you are not a patient here or have not signed up for your own personal MyUofMHealth portal yet, use this online form to request access.
If you represent an adult that you are legally responsible for, you can request proxy access for them on our MyUofMHealth Parent/Guardian/DPOA Proxy Access Request Form page and choose the "Legal Guardian" or "DPOA for Healthcare" option for Relationship to Patient.
As a parent you have the right to access your child’s health information and can elect to request and receive copies of medical records in your MyUofMHealth portal. To comply with state and federal privacy laws, parents of children ages 13-17 years old are assigned partial access that includes the immediate sharing of appointments and notes from most specialty departments.

Steps to request medical records from/to your MyUofMHealth portal:
- Log in to your personal MyUofMHealth portal.
- Switch to your child’s profile.
- Search Your Menu for Medical Record Request Form.
- Complete and submit online form.
