An Inpatient Heart Failure Service at St. Joseph Mercy Chelsea Hospital
The Chelsea Heart Failure Program has been developed to provide heart failure patients with all the resources they need for success and to minimize the chance for readmission.
During the first 30 days following discharge, heart failure patients face a critical time in their recovery. Having close monitoring and support, as well as education about their health process is key to an optimal outcome.
Patients eligible for this program must meet certain criteria to be admitted to the program.
