Chest Masculinizing Surgery
Plastic surgeons at Michigan Medicine are proud to provide mastectomy and chest reconstruction surgery. To access this service, refer to the process below:
- Fill out and return the Service Inquiry Form for CGSP
- Meet with a fully licensed mental health clinician who is knowledgeable about gender identity, and who is able and willing to write a letter of support for this surgery. Contact us if you need a referral to such a clinician.
- Ensure that your therapist sends a letter of support for mastectomy and chest reconstruction to: CGSP, 4250 Plymouth Rd, Ann Arbor, MI 48109. The letter may also be faxed to 734-998-2152. The letter must come directly from the therapist to CGSP via mail or fax. If your therapist does not have experience writing letters of support for medical intervention for transgender people, please encourage your therapist to contact us before writing the letter.

The following insurance providers have a policy that requires people with these insurance plans to obtain a 2nd referral letter from a licensed mental health clinician before they will grant insurance coverage for mastectomy. If your insurance plan is one of the insurance providers listed below, you must obtain a 2nd referral letter for mastectomy before CGSP can refer you to Plastic Surgery.

- McLaren
- Ambetter by Meridian - This is a plan available through the ACA Individual Marketplace. It does not include Meridian Medicaid, Healthy Michigan Plan, or MI HealthLink.
- Some out-of-state Blue Cross Blue Shield - This does not include Blue Cross Blue Shield of Michigan. If you have an out-of-state Blue Cross Blue Shield plan, please contact our office at 734-998-2150 to clarify your insurance company’s requirements.
- Potentially other providers
Breast Augmentation Surgery
Plastic surgeons at Michigan Medicine are proud to provide breast augmentation surgery. To access this surgery, refer to the process below:
- Fill out and return the Service Inquiry Form for CGSP
- Meet with a fully licensed mental health clinician who is knowledgeable about gender identity, and who is able and willing to write a letter of support for breast augmentation surgery. Contact us if you need a referral to such a clinician.
- Ensure that your therapist sends a letter of support for breast augmentation to: CGSP, 4250 Plymouth Rd, Ann Arbor, MI 48109. The letter may also be faxed to 734-998-2152. The letter must come directly from the therapist to CGSP via mail or fax. If your therapist does not have experience writing letters of support for medical intervention for transgender people, please encourage your therapist to contact us before writing the letter.
Please note that breast augmentation surgery is not currently covered by insurance. Both the consultation with a plastic surgeon and the surgery are self-pay.
Please note that before you can schedule your consultation appointment with Plastic Surgery, you will need to be completely smoke-free and nicotine-free for at least 2 months. We cannot schedule your consultation appointment if you smoke or vape any substance or use/consume any nicotine-containing products.
