Participation in Transgender Program

Participation in the UMHS-CGSP can involve one or more steps. Each step is designed to actively include the individual in their treatment and care decisions.

Step 1 – Enrollment To begin the process of becoming a part of the program, please download the enrollment form here, complete the form, and return it to us via email ([email protected]) or fax (734) 998-2152. Once your enrollment form is received, you will be contacted by someone from the CGSP office to determine more specifically how we can be of service to you.  If you cannot download the form, you may also email and request that it be emailed or mailed to you.

Step 2 – Learning About Your Needs This process consists of at least one telephone conversation with a member of the UMHS-CGSP Team. Team members include administrators and social workers or psychologists with special interest and training in transgender healthcare. While we only refer within the UMHS CGSP for some of these services, referrals are also made to our affiliates throughout Michigan when appropriate. The CGSP team will become your advocates in the program. They will outline what is needed for you to reach your goals, and during this process, they will direct you toward resources, such as mental health, medical, or surgical care, or for ancillary services, such as electrolysis or speech/vocal therapy. They will also be able to refer you to CGSP support groups, through which you can connect with others.

What if I’ve already started transitioning? If you are already in transition (that is, you have been evaluated and/or treated in another mental health or medical facility, or you have educated yourself and are proceeding on your own toward your goals), we can work with you or your current providers to assist you in moving forward. In that case, once you are enrolled we will make the standard telephone contact (in Step 2 above) to determine your needs and how we can help.
